Completing and submitting this form will reserve a spot in the 2010 rowing camp. After registering, you must print, complete, and sign the medical history and release and consent forms. Mail those forms, a copy of the athlete's insurance card (front and back), and a check for $200 (non-refundable deposit) made out to "Cavalier Rowing Camp" to:
